Cardiovascular, Stroke, Renal and Diabetes

The Cardiovascular (CVD) Network covers the four clinical areas of Cardiac, Stroke, Renal and Diabetes.

The purpose of the CVD Strategic Clinical Network (SCN) is to bring together a wide range of stakeholders to work across geographical and organisation boundaries to support equity of access to high quality, evidence based care.

The Cardiovascular Outcomes strategy published in 2013 brings together a family of diseases including the four outlined above, and provides a framework within which the Thames Valley CVD SCN Work Plan has been created. This includes cross-cutting themes such as end of life care where appropriate, and aims to incorporate the five NHS Outcomes Framework Domains. The work plan is monitored by the CVD Steering Group and actions are pursued through a number of Network and Locality groups with project support.

Healthcare professionals can access local cardiac, diabetes, renal and stroke data via the Cardiovascular Intelligence Network.
