Transition from Children to Adult Health services

 One of the projects to be focussed on by the Children’s network is Transition from Children to Adult Health Services.

 What is transitional care?

“ a purposeful, planned process for adolescents with chronic physical and mental conditions as they move from child-centred to adult orientated health care”
(Blum et al 1993)

polly schofieldHi I am Polly and I am a Transition Project Nurse a post funded by the Thames Valley Strategic Clinical Network and hosted at the Royal Berkshire Hospital. Part of my work at the Royal Berkshire Hospital is to support specialties to implement the ‘Ready Steady Go’ Transitions programme. The programme, which was developed at the University of Southampton, aims to equip young people with the skills and knowledge to make a successful transition from paediatric to adult services.  You can read more about Ready Steady Go here

 

Update: December 2015

The TVSCN transition project has been going for one year now and my colleague Katie King has done a fantastic job over the last 6 months, going out to acute trusts across the Thames Valley delivering training on transition and the Ready Steady Go program.  Many specialties have taken up this training opportunity and we are confident that those professionals now have access the tools and resources to get Ready Steady Go implemented in their areas.

We held an event in December to showcase some of the fantastic transition service development work that is going on across the region.  We were delighted to have some fantastic speakers, including two members of the University Southampton Hospitals transition steering group, who developed Ready Steady Go. Next steps:

Over the next year, I will be continuing to work with hospitals across the Thames Valley to support trusts in developing their transition services.  This support will include advising trusts on how to set up a transition steering group, and, where required, providing these steering groups with strategies to ensure the smooth implementation of Ready Steady Go.  I can also support trusts with the development of their Trust Transition Policy and the different elements of this that will ensure the hospital is compliant with the upcoming NICE guidelines on transition.
